Permission to Travel Notification - Used anytime your team is traveling outside the state of Kentucky

Guest Player Notification - Used anytime your team wishes to use a player that is not listed on your official roster for a tournament.

**You will no longer need a user name or password.  These forms must be completed in their entirety.  Once submitted, the system will display a confirmation link where your documentation can be retrieved as well as an email confirmation.  If you have any issues or need a copy of your form, please email stateoffice@kysoccer.net.
   

Permission to Host Notification (Microsoft Word doc) - Used anytime your team is hosting games for teams that are not members of Kentucky Youth Soccer but are members of any other US Soccer Federation member.
