The first time you use the system, you will be asked to complete a form with your team’s information. Once this is done, you will be given a password to use in the future. You will not need to provide the information for your team again as long as you remember your password.
Permission to Travel Notification
Used anytime your team is traveling outside the state of Kentucky.
Guest Player Notification
Used anytime your team wishes to use a player that is not listed on your official team roster for a match or tournament.
Permission to Host Notification
Used anytime your teams are hosting games for teams that are not members of Kentucky Youth Soccer but are members of any US Soccer Federation member.
Notification to Play Games Against Non KYSA Teams
Used anytime your teams are playing games against teams that are not members of Kentucky Youth Soccer or any US Soccer Federation Member.
The forms must be completed in their entirety. Upon completion of the forms you will be given a confirmation number and a copy of the completed form will be displayed on your screen. You can go back at anytime and display or print these forms. No one can make changes to these forms. Once completed, you and your club will be emailed a copy of the confirmation for record keeping purposes. Your club should keep a copy of these forms, rosters and guest player forms on file for at least 5 years.
